Job summary

Table Rock Post Acute, part of Cascadia Healthcare, seeks a Registered Nurse to serve as Resident Care Manager. You will develop and evaluate health care plans, lead clinical teams, and ensure high-quality patient care across the facility.

Responsibilities include communicating plans with families and physicians, supervising care staff, and maintaining accurate medical records while delivering compassionate, person-centered care in a 24-hour skilled nursing setting in Medford, Oregon.

Qualifications

• RN nursing degree from an accredited school.
• Current RN license.
• CPR certification.

Responsibilities

• Evaluates updates resident health care plans to achieve person-centered care.
• Conducts assessments for new residents/patients.
• Communicates with families and physicians about treatment plans.
• Maintains accurate medical records and ensures completeness of documentation.
