La Grande Post Acute Rehab in La Grande, OR, is seeking a full-time MDS Coordinator RN to lead the MDS/RAI processes with a strong focus on accuracy and regulatory compliance. This role blends clinical leadership with hands-on MDS responsibilities to support quality resident care.

The position offers 40 hours per week, a 24-hour MDS component, and a 16-hour Resident Care Manager portion, plus relocation assistance, full medical/dental/vision benefits, and opportunities for leadership growth.

Qualifications

• Active, unencumbered RN license in Oregon (or eligible for reciprocity).
• Experience in LTC/SNF environments recommended.
• Hands-on familiarity with MDS/RAI process preferred.
• Experience with PCC (PointClickCare) a major plus.
• Organized, collaborative, and able to teach others.

Responsabilités

• Champion the MDS/RAI process and ensure accurate documentation for compliance.
• Split role with 24 hours for MDS coordination and 16 hours for Resident Care Manager duties.
• Mentor floor staff, collaborate on care plans, and engage with residents and families.

This full-time (40 hours/week) position keeps your work dynamic and engaging by perfectly blending clinical strategy with hands-on leadership:

• 24 Hours - MDS Coordinator: Champion the RAI process, ensure accurate documentation, and protect our compliance standard.

• 16 Hours - Resident Care Manager (RCM): Mentor floor staff, collaborate on care plans, and build meaningful relationships with residents and families.

What You Bring to the Team

• An active, unencumbered RN license in Oregon (or eligible for reciprocity).
