La Grande Post Acute Rehab is seeking an MDS Coordinator RN to lead the RAI process and mentor care teams in La Grande, Oregon. This full-time role blends clinical leadership with hands-on MDS duties, ensuring accurate documentation and compliance while supporting residents and families.

Qualifications

• Active, unencumbered RN license in Oregon.
• Hands-on familiarity with MDS/RAI process (preferred).
• Experience in Long-Term Care or Skilled Nursing Facility.
• Experience with PointClickCare (PCC) is a major plus.

Responsibilities

• Split role with 24 hours as MDS Coordinator focusing on RAI accuracy and compliance.
• 16 hours as Resident Care Manager mentoring floor staff and care plans.
• Lead and develop staff; ensure resident and family communication.
• Collaborate with interdisciplinary team to improve outcomes.

• 24 Hours - MDS Coordinator: Champion the RAI process, ensure accurate documentation, and protect our compliance standard.

• 16 Hours - Resident Care Manager (RCM): Mentor floor staff, collaborate on care plans, and build meaningful relationships with residents and families.

What You Bring to the Team
