Job summary

Pacific Retirement Services, Inc. in Medford, OR is seeking a Resident Care Manager/MDS Coordinator (RN) to lead the delivery of person-centered care for both short-term and long-term residents.

The schedule is Monday–Friday, 9am–5pm, and the salary will be based on experience. You will work closely with an interdisciplinary team to coordinate admissions, assessments, care planning, and regulatory readiness while mentoring nursing staff and ensuring high standards of care and resident

Qualifications

• Minimum 1 year of professional nursing in a skilled nursing setting.
• RN license from an accredited program, school or university.
• Prior supervisory experience in a similar setting including performance management and staff oversight.
• Ability to work independently and make sound judgments on resident care.
• Knowledge of state, federal and local long-term care regulations.
• Current CPR certification.
• Experience with electronic medical records systems.

Responsibilities

• Lead and inspire a high-performing nursing team.
• Advocate for resident rights and deliver resident-centered care.
• Coordinate care across physicians, nurses, therapists, and other team members.
• Oversee admissions, assessments, care planning, conferences, and discharge planning.
• Monitor resident conditions, orders, incidents, and clinical documentation.
• Drive quality improvement initiatives and regulatory readiness.
• Collaborate with families to maintain open communication and care planning.
• Support MDS coordination, PDPM and OBRA compliance, utilization review processes.
• Mentor and develop nursing staff through coaching and training.
• Provide on-call leadership support as needed.
• Perform other duties as assigned.
