Day shift work from 08:00 AM to 04:30 PM, on-site. You will assess needs, craft care plans, collaborate with a multidisciplinary team, and document in EPIC to track progress.

Qualifications

• Active RN license.
• Minimum of 1 year experience in Case Management.
• Familiarity with EPIC is preferred.
