Job summary

Origin Travel Nurses is seeking a dedicated Lead Case Manager for our inpatient rehabilitation program. You will oversee case management activities, coordinate care plans with a multidisciplinary team, and serve as the liaison between patients, families, and clinicians.

The role requires current RN or Social Work licensure (or related therapy qualifications), CCM/ARN/ACM certification preferred, and at least 2 years of inpatient case management experience with prior leadership experience.

Qualifications

• Current RN or Social Work licensure, or qualifications as a Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
• Certification in Case Management or Rehabilitation (e.g., CCM, ARN, ACM, RN-BC) preferred.
• Minimum of 2 years in social work or case management in an inpatient setting is highly preferred.

Lead Case Manager - Inpatient Rehabilitation

Job Description

We are seeking a dedicated and experienced Lead Case Manager for our state-of-the-art inpatient acute rehabilitation facility. This role is essential in supporting individuals who are recovering from serious injuries or illnesses through tailored rehabilitation programs.

Responsibilities

• Oversee and coordinate case management activities for patients throughout their recovery journey.
• Collaborate with healthcare professionals to develop and implement individual patient care plans.
• Act as a liaison between patients, families, and healthcare teams to ensure effective communication.
• Provide support and education to patients and families regarding treatment plans and rehabilitation goals.
• Assist in discharging patients with comprehensive follow-up plans and resources.

Schedule

This position is a full-time role, with a schedule from 8:00 AM to 4:00 PM, Monday through Friday.

Required Experience / Certifications / Licensure

• Current Registered Nurse or Social Work licensure, or qualifications as a Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
• Certification in Case Management or Rehabilitation is preferred (e.g., CCM, ARN, ACM, RN-BC).
• Minimum of 2 years of social work or case management experience in an inpatient setting is highly preferred.
• Experience in acute or rehabilitation hospital settings is strongly preferred.
• Previous leadership or supervisory experience is strongly preferred.
