Job summary

VyTalent Solutions is seeking a RN: Case Manager for a Home Health contract in Colorado Springs, CO. The position requires an active RN license and a degree in nursing or related field, with case management certification preferred.

You will assess high-risk members, coordinate care plans with clinicians, and manage transitions with a multidisciplinary team. The role is a 13-week traveler contract, scheduled Mon-Fri 8a-5p, with a pay range shown and expectations to maintain detailed clinical

• Current, unrestricted state Registered Nurse license.
• Associate or bachelor’s degree in nursing or related field.
• Ability to analyze complex medical information and make rational decisions.
• Case Management certification.

Responsibilities

• Perform PRN/focused and routine assessments of high risk members.
• Collaborate with primary care providers to develop and implement individualized care plans.
• Address, identify, and reassess appropriate levels of care, including non-medical support to ensure treatment compliance.
• Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
• Keep detailed records of clinical, functional, and fiscal outcomes during the management process.
