ARMStaffing is seeking a dedicated Registered Nurse (RN) for Case Management in Texas. You will assess psychosocial, medical, and discharge needs, coordinate care plans, and collaborate with a multidisciplinary team to evaluate progression throughout the episode of care.

This position involves day shift 8:00 AM - 4:30 PM with rotating weekends, and requires strong communication and advocacy for patients and families.

Qualifications

• Active RN license issued in Texas.
• Minimum of 2 years of acute case management experience.

Responsibilities

• Assess patients and families comprehensively.
• Coordinate discharge planning and care plans with the multidisciplinary team.
• Drive discharge progression using InterQual criteria.
• Collaborate with Licensed Social Workers to address psychosocial needs.

• Collaborate with Licensed Social Workers to address psychosocial needs.
• Provide education, counseling, and advocacy for patients and families.

Required Experience / Certifications / Licensure

• Active RN License in Texas.
• A minimum of 2 years of acute case management experience.
