Job summary

ARMStaffing is seeking a Case Manager to oversee utilization reviews, resource management,discharge planning, treatment plan management, and medical record documentation.

The role emphasizes patient safety and collaborative care with patients, families, and the interdisciplinary team, with day shift, 5 days a week.

Qualifications

  • Current licensure in a clinical discipline per state guidelines (RN, RT, or Social Worker preferred).
  • Associate's Degree in a healthcare field; Bachelor’s degree preferred.
  • Two years of case management and discharge planning experience in a critical care setting.
  • Working knowledge of the insurance industry and government reimbursement processes.

  • Identify and address patient problems affecting their ability to participate in their treatment plan.
  • Assist patients and families in utilizing hospital resources to remove barriers related to their healthcare situation.
  • Collaborate with the interdisciplinary team to manage and direct a clinical plan of care aimed at ensuring safe and timely discharge.
  • Interact appropriately with patients, tailoring communication to their age and understanding.
  • Refer patients to outside agencies for social issues as required by state regulations.
  • Notify the Director of Case Management (DCM) or CEO of major conditions, changes, and emergency situations.
  • Conduct team and family conferences in accordance with policy and procedure.
  • Participate in facility-wide operations improvement and needs identification.
  • Ensure compliance with medical necessity review criteria for admissions and continued stays.
  • Maintain an up-to-date community resource system for patient families.
  • Document case management activities accurately and in a timely manner.

Job description

This role emphasizes patient and employee safety and promotes a collaborative approach to care that involves the patient, family, and interdisciplinary team.

Responsibilities

Required Experience / Certifications / Licensure
  • Current licensure in a clinical discipline per state guidelines (Registered Nurse, Respiratory Therapist, or Social Worker preferred).
  • Associate's Degree in a healthcare field (Bachelor’s degree preferred).
  • Working knowledge of the insurance industry and government reimbursement processes.
  • Two years of experience in case management and discharge planning in a critical care setting.
Schedule

Day Shift, 5 days a week, 8-hour shifts.
