Hospice of the Midwest in Edina, MN is seeking a passionate Hospice Admissions RN to join our team. You will provide expert, patient-centered care at a critical time in people’s lives. The role offers 1:1 patient care, flexible scheduling, and opportunity to work independently with strong team support.

Applicants should be a graduate nurse with valid MN license and hospice experience. This position covers Hennepin, Carver and Sherbourne counties with a commitment to compassionate home-based care

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care.
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
