Suncoast Hospice, part of Empath Health, is seeking an RN Case Manager to provide compassionate end-of-life care across Pinellas County, with responsibilities in home and facility settings. The role emphasizes care coordination, family education, and adherence to plan of care.

The ideal candidate holds an approved nursing program credential and a Florida RN license, plus at least one year of acute-care experience; hospice experience is preferred.

Suncoast Hospice, a part of Empath Health is seeking a caring and motivated RN Case Manager to work with us in Pinellas County while providing exceptional care on the following Coverage territory Pinellas county working in both home/facility team. What You'll Need:

• Applicants must be a graduate of an approved nursing program and own a current Florida RN license.
• Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a hospice RN preferred.
• Valid Florida State Driver’s License This is only required if colleague chooses driving herself/himself as mode of transportation when reassigned to work at another facility. This is not a requirement if the colleague chooses an alternative means of transportation when reassigned to another facility based on business needs. Regardless of the mode chosen, both are at the colleague’s own expense, with the exception of mileage reimbursement eligibility when traveling between facilities while on company time.
