Job summary

Hospice of Marion, a Part of Empath Health, seeks a compassionate Hospice RN to provide in-home and facility-based end-of-life care in Marion County. You will perform histories, assessments, patient teaching, and collaborate with the IDT to plan personalized care.

You will supervise LPNs and aides, monitor medications, document accurately, and educate families about hospice care. A valid Florida RN license is required.

Qualifications

  • Associate’s degree in nursing or equivalent.
  • Current Florida RN license and valid driver's license.

Responsibilities

  • Perform age-specific assessments and coordinate care with the IDT.
  • Educate patients and families; tailor teaching to needs.
  • Document per policy; ensure timely, compliant records.
  • Supervise LPNs and Home Health Aides and evaluate care.
  • Administer and monitor medications as prescribed.

Job description

Hospice of Marion, a Part of Empath Health, is seeking a Hospice RN to join our team to visit patients and provide clinical, physical, educational and emotional support after-hours in in the patient homes, long term care facilities and/or assisted living facilities in Marion County. Schedule: Mon, Tues, Wed, Fri ; 2pm - 12am Since 1983, Hospice of Marion County has been honored to serve families across Ocala and Marion County with compassionate, expert end-of-life care. As part of Empath Health, we help neighbors with advanced illness live as comfortably and meaningfully as possible, while supporting their families and caregivers with respect, guidance, and dignity.

What You’ll Do
  • Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.
  • Identifies age-specific patient and caregiver learning needs, motivations, barriers, abilities and preferences and utilize teaching strategies/instructions and follow-through to match.
  • Documentation is compliant with all organizational standards including productivity, timeliness and adheres to relevant policies and practice guidelines.
  • Following a patient’s death, assists with closure contact and support in collaboration with IDT.
  • Directs and supervises the care provided by the LPN and Home Health Aide following agency policy.
  • Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.
  • Educates and represents the community regarding hospice care philosophy and concept through a case by case consultation with persons in the community or educational inservice or seminar as assigned.
  • Provides education to IDT members regarding nursing aspects of patient/family care.
  • Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician.
  • Uses age-specific care practices and provide appropriate patient/family teaching.

What You’ll Need

Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; Minimum of one year related experience and/or training; or equivalent combination of education and experience.

Licensure
  • Valid Florida license as a Registered Nurse
  • Valid Florida State Driver’s License

Prefer one (1) year experience in an acute or post acute care setting

Preferred experience caring for terminal patients and others with life threatening illnesses.

For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
