Job summary

BAYADA Home Health Care is seeking a Full Time Registered Nurse Preceptor to train and mentor newly hired RNs and LPNs in adult/geriatric home health. The role requires travel across the Wilmington, MA area with patient care responsibilities and ongoing clinical support.

The RN Preceptor will provide on-site and field-based training, perform assessments, and ensure high-quality care while maintaining a caseload. Medicare home health and OASIS experience preferred.

Qualifications

• Active MA RN license in good standing.
• One+ year hands-on medical experience in adult/geriatric populations.
• Experience in home health, Medicare OASIS preferred.

Responsibilities

• Identify subtle changes in patient condition via head-to-toe assessments.
• Provide triage and manage urgent clinical situations in the home.
• Educate and mentor newly hired RNs and LPNs during onboarding.
• Perform medication reconciliation, teaching, and safety monitoring.
• Document wound assessments, care, and progress in the record.
