Job summary

Baystate Health is seeking an experienced Registered Nurse to optimize patient placement and throughput within the hospital system. You will integrate clinical and financial considerations to place patients in the appropriate care setting and bed, collaborating with care teams to ensure timely admissions and transfers.

Responsibilities include evaluating level of care, coordinating bed assignments, and guiding care plans while maintaining quality standards and hospital policies.

Qualifications

• 5 years of clinical nursing experience
• Experience in ED/ Critical Care preferred
• Bachelor's Degree in Nursing required
• Registered Nurse – State of Massachusetts certification

Responsibilities

• Facilitate and coordinate patient bed placement based on level of care and clinical needs.
• Perform utilization analysis on admission using guidelines and nursing judgment.
• Act as throughput decision-maker for admissions, transfers, and bed assignments.
• Communicate level-of-care changes to care team and coordinate care plans.
• Participate in quality management initiatives and data collection for throughput metrics.
• Identify patients needing physician review for admission/transfer and escalate when needed.
• Coordinate Transfer Center calls with providers and plan bed assignments after acceptance.
• Collaborate with clinical supervisor to resolve throughput barriers on the floors.
• Work with unit staff to optimize bed placement and capacity against demand.
• Monitor in-house patient needs for triage capability and collaborate with MDs.
• Maintain required program compliance and education records.
• Serve as resource for Transfer Center software and related applications.
• Provide alternatives for patient care locations when primary bed is unavailable.
• Coordinate bed-placement communications with the Patient Placement Assistant.
