Job summary

FMOL Health is seeking a Patient Placement RN to coordinate admissions, transfers, and discharges, ensuring efficient bed utilization and patient throughput. The role collaborates with physicians, nursing staff, and ancillary teams to promote quality care and satisfaction.

The RN will triage referrals from ER and other providers, match patients to HMS teams, and maintain real-time bed and census data to support timely care and post-discharge coordination.

Qualifications

• 2 years nursing experience in an acute setting such as critical care, hospital medicine, or patient flow.
• Current Louisiana RN license required.
• Degree or equivalent experience as stated above.

Responsibilities

• Coordinate patient admissions, transfers, and discharges for efficient throughput.
• Triages referrals from ER physicians and other providers to HMS or LSU teams.
• Match patient needs with the appropriate HMS environment and capacity.
• Maintain real-time awareness of bed availability and census.
• Ensure placement requests are prioritized and processed timely.
• Publish HMS assignments daily with current census and projections.
• Coordinate patient care handoffs and ensure continuity of care.
• Collaborate with hospital staff to promote patient safety and satisfaction.
• Escalate throughput issues and resolve conflicts between departments.
