Job summary

Mercyhealth is seeking an RN Patient Navigator to coordinate patient care, improve outcomes, and support efficient resource use. The role includes case management, early assessment, and post-acute referrals, with a focus on preventing readmissions and ensuring smooth transitions across care settings.

This position requires a BSN or enrollment in a BSN program, active RN license in the practice state, and ability to educate patients and families.

Qualifications

• Graduate of an accredited nursing program.
• BSN or enrollment in a BSN Completion Program.
• Current Registered Nurse License in the state of practice.

Responsibilities

• Provides case management, advocacy, and care coordination to hospital patients to ensure seamless transitions.
• Conducts early patient assessment to identify care coordination needs.
• Creates individualized plan for continuing care needs.
• Makes referrals to post-hospital providers using community resources.
• Collaborates with the healthcare team to ensure timely discharge and complete follow-up.
• Educates patients and families about chronic disease conditions.
• Ensures core measures are met prior to discharge.
• Identifies and intervenes in financial risk to patient and organization.
