Job summary

Requirements include a nursing baccalaureate, RN licensure in Iowa, and BLS; case management and insurance knowledge are valued for effective transitions and a collaborative team environment.

Qualifications

• RN licensure in Iowa and BLS certification are required.
• Experience in case management and discharge planning preferred.
• Understanding of Medicare/Medicaid and insurance processes is advantageous.

Responsibilities

• Review complex patient status and coordinate care across multiple programs.
• Communicate with physicians and clinical staff daily on plan of care and progress.
• Facilitate care conferences and ensure accurate admission status using InterQual criteria.
