Baylor Scott & White Health is seeking a Patient Navigator RN in Texas to coordinate patient care with the medical team, plan and evaluate care, and advocate for patients. The role emphasizes autonomous work, patient advocacy, and collaboration with clinicians.

You will schedule outpatient tests, accompany patients through the hospital, maintain nursing records, and participate in care conferences and improvement initiatives.

Competenze

• Bachelor's degree in nursing.
• Active RN license.
• Minimum 3 years of relevant nursing experience.
• BLS certification or willingness to obtain within 30 days.

Mansioni

• Coordinate patient care and plan, implement, and evaluate with the medical team.
• Schedule outpatient tests and maintain communication with care team.
• Guide patients and families through hospital areas and admissions process.
• Attend case conferences and participate in quality improvement projects.
• Maintain accurate nursing records and update the Navigator database.
• Advocate for patients and provide education and resource navigation.
