Job summary

Baylor Scott & White Medical Center – Round Rock is seeking a Patient Navigator RN to support seamless patient care coordination. You will assess patient needs and help plan, implement, and evaluate care in collaboration with the medical team, working both autonomously and with colleagues.

In this role you will advocate for patients, educate families, schedule tests, maintain nursing records, and guide patients through the hospital.

Qualifications

• Bachelor's degree required.
• 3+ years of relevant RN experience preferred.
• CA Registered Nurse with active license; BLS certification or ability to obtain within 30 days.

Responsibilities

• Care for patients by evaluating health per doctor's orders and planning care.
• Schedule outpatient tests and coordinate with the healthcare team.
• Visit doctors, support patient-office communication.
• Collaborate with professionals for optimal patient care.
• Conduct follow-ups at 3 months and 1 year.
• Maintain nursing records and update Patient Navigator database.
• Attend case conferences and participate in community service.
• Contribute to service improvement projects.
• Welcome patients and families, provide facility info and directions.
• Guide patients and families through the hospital to locations.
