Baylor Scott & White Health in Temple, TX seeks a Patient Navigator RN to coordinate patient care across the medical team, assess needs, and advocate for patients. You will schedule outpatient tests, guide patients through the hospital, maintain nursing records, and participate in case conferences, with focus on compassionate care and efficient care transitions.

Qualifications

• Bachelor's degree in Nursing required.
• At least 3 years of relevant experience for the role.
• Registered Nurse license required.
• Willingness to obtain Basic Life Support (BLS) within first 30 days.

Responsabilités

• Evaluate patient health and create a care plan in coordination with the medical team.
• Schedule tests and follow up with the care team.
• Welcome patients and families, guide them through the hospital.
• Maintain accurate nursing records in the navigator system.
• Attend case conferences and participate in community service events.
• Provide patient education and advocacy.
