Job summary

Baylor Scott & White Health in Round Rock, TX seeks a Patient Navigator RN to coordinate patient care across the medical team, advocate for patients, and serve as educator and liaison. This full-time day position requires autonomous work and collaboration to plan, implement, and evaluate care as patients move through tests and treatments.

You will schedule outpatient tests, maintain up-to-date nursing records, participate in conferences, and help patients navigate the hospital environment,

Qualifications

• Bachelor's degree required.
• Licensed Registered Nurse in good standing.
• Minimum 3 years of relevant RN experience.
• BLS certification or willing to obtain within 30 days.

Responsibilities

• Evaluate patient health and create a care plan with the medical team.
• Schedule outpatient tests and coordinate with the healthcare team.
• Guide patients and families through the hospital and provide directions.
• Maintain nursing records and update the Patient Navigator database.
• Participate in case conferences and community service events; support continuous improvement.
• Follow up with patients at 3 months and 1 year.
