Job summary

Baylor Scott & White Health in Temple, TX seeks a Patient Navigator RN to coordinate patient care with the medical team, assess patient needs, and implement and evaluate care plans. You will act as advocate, educator, and coordinator for patients, ensuring seamless service delivery and quality outcomes.

You will work with a multidisciplinary team, scheduling tests, communicating with patients and doctors, and maintaining thorough nursing records to support the care plan.

Qualifications

• Bachelor's degree in Nursing required.
• At least 3 years of relevant nursing experience.
• Registered Nurse license in good standing.
• BLS certification required within first 30 days.

Responsibilities

• Assess patient needs and help plan, implement, and evaluate care with the medical team.
• Coordinate patient care and advocate for patients and families.
• Serve as educator and coordinator for patients and caregivers.
• Care for patients by evaluating health and creating a care plan.
• Schedule outpatient tests and maintain ongoing communication with the care team.
• Visit doctors, support patient-office communications, and document progress.
• Collaborate with professionals for optimal patient outcomes.
• Attend case conferences and participate in community service events.
• Guide patients and families through the hospital and provide directions.
