WakeMed Health & Hospitals in Cary, NC seeks a Patient Navigator, RN to guide patients through care pathways, explain treatment options, educate and advocate for patients and families, and coordinate communication across disciplines.

The role involves visiting patients in hospital, following up after discharge, and connecting families with community resources; requires RN licensure, BSN and preferred MSN, and about 5 years acute care nursing experience.

Formación

• Registered Nurse (RN) licensure required.
• Bachelor's degree in nursing required; master's degree preferred.
• Minimum 5 years of nursing experience in acute care.

Responsabilidades

• Guide patients and families through care pathways with a clinical lens.
• Explain treatment options and provide advice and support to patients.
• Educate, advocate for, and encourage patients and families.
• Coordinate communication across disciplines and care teams.
• Connect patients with appropriate care and community resources.
• Visit patients in hospital or recovery and follow up after discharge.
