Huntsville Memorial Hospital is seeking an RN Navigator for Case Management to assess, plan, implement, and evaluate population-based nursing care. The role focuses on reducing readmissions, coordinating follow-up care, and connecting patients with community resources to support ongoing health needs.

The ideal candidate is a graduate nurse with active license or eligibility, strong communication, and the ability to work autonomously within an interdisciplinary team.

Qualifications

• Strong analytical, data management, organizational and time management skills.
• Knowledge of teaching/learning theories and ability to educate patients and families.
• Experience in case management or navigation is preferred.

Responsibilities

• Assist patients and families in understanding diagnosis and treatment options.
• Facilitate post-hospital follow-up appointments with physicians and other care providers.
• Collaborate with community resources to address health, psychosocial, or financial needs.
