Job summary

Huntsville Hospital is seeking a clinically oriented nursing professional to join a collaborative provider practice. You will initiate patient care assessments, gather vital signs, prepare exam rooms, and collect specimens to support quality patient care.

The role emphasizes coordinating care for patients with chronic disease, supporting preventative visits, and coaching patients in self-management to reduce health risks.

Qualifications

• Graduate of Accredited School of Nursing with current Alabama licensure.
• Experience preferred; new graduates accepted.
• BLS required before the conclusion of the orientation period.

Responsibilities

• Assess problems/conditions during workup and document in medical records.
• Assist provider in examination, treatment and referral; prepare paperwork/equipment; injections and point-of-care testing; provide education and referrals.
• Order and maintain adequate supply levels.
• Train new clinical staff within the clinic.
• Coordinate clinical coverage with network leadership.
• Stratify patient population by risk to manage appointments and maximize staff utilization.
• Ensure tracking of test results, medications, missed appointments, and follow-ups.
