Job summary

Huntsville Hospital is seeking a Case Manager to assist in the development, planning, coordination and administration of utilization review and discharge planning. The role involves reviewing medical records to determine medical necessity for admission, continued stay, and initiating discharge planning.

The Case Manager must work independently, handle multiple assignments, manage time well, communicate effectively, and perform under pressure in a fast-paced hospital setting.

Qualifications

• RN required with at least one year clinical experience on a med/surg unit.
• Three years of case management or utilization management experience preferred.
• Bachelor's or higher in a related field is preferred.

Responsibilities

• Conduct concurrent case management and discharge planning activities.
• Identify and address barriers to discharge.
• Coordinate with patients, families, physicians, and community resources.
• Facilitate referrals to post-acute providers.
• Participate in interdisciplinary rounds.
• Support patient progression and regulatory/compliance initiatives.
• Document case management interventions.
• Collaborate with the interdisciplinary team to ensure safe transition of care.
