Northwestern Medicine seeks a Patient Flow Coordinator, RN (PFC, RN) to support fast-paced patient care within the ICC. You will perform comprehensive nursing assessments, administer care, and coordinate patient flow while collaborating with physicians and staff.
The role requires a BSN, a current Illinois RN license, and BLS certification, with preference for emergency department or urgent care experience and Epic proficiency.
Formación
- BSN required or equivalent nursing degree.
- Current Illinois RN license is mandatory.
- BLS certification required.
Responsabilidades
- Maintain safe and clean work environment and ensure OSHA compliance.
- Perform comprehensive nursing assessments and document in medical records.
- Provide direct patient care and consultation using clinical knowledge.
- Coordinate patient education and discharge planning within ICC guidelines.
- Collaborate with clinicians and departments to coordinate patient care.
- Triage calls and results and communicate plans with patients via MyChart or phone.
- Respond to medical emergencies per hospital protocols.
- Serve as resource and role model for staff and clinicians.
