Job summary

Citadel Healthcare is seeking a Registered Nurse (RN) to join our team in Rockford, IL. The RN will be responsible for assessing, implementing, and evaluating plans of care for residents, ensuring safety and quality of care in a long-term care setting.

The role requires an active RN license in Illinois, CPR certification, and a nursing degree or diploma. Prior long-term care experience is preferred, with strong communication and multitasking skills essential to success.

Qualifications

• CPR certification required.
• Nursing degree or diploma from an accredited college or university.
• Current license to practice as a Registered Nurse in the state.
• 1 year of experience preferred; long-term care experience preferred.

Responsibilities

• Provides licensed nursing care to assigned residents as ordered by physician and per standards.
• Receives and transcribes physician orders to resident charts.
• Administers medications and treatments as ordered; maintains MAR and care plans.
• Counts controlled drugs with another nurse at shift change; reports discrepancies.
• Admits, transfers and discharges residents to meet individual needs; ensures safety.
