Job summary

Behavioral Health Network, Inc. is seeking a Registered Nurse for its PACT team in Western Massachusetts. You will provide direct and indirect nursing services, coordinate physical-health assessments, and support integrated medical and psychiatric care.

The role requires a current RN license, a related degree, and knowledge of MAP; six months in human services is preferred. You may work weekends/evenings as needed in a mission-driven organization with strong HEART values.

Qualifications

  • Current, unrestricted RN license and required nursing certifications.
  • At least six months of experience in human services preferred.
  • Knowledge of MAP requirements, medication management, physical-health assessment and care coordination.

Responsibilities

  • Provide direct and indirect nursing services to individuals in PACT.
  • Assess physical-health needs and refer to medical providers; integrate medical and psychiatric treatment.
  • Follow MAP policies and document health- and medication-related data.
  • Manage medications, pharmaceuticals, and related clinical workflows with the team.
  • Develop, implement, review and document treatment plans and recovery goals.
  • Serve as primary case manager for assigned caseload when requested; complete assessments and treatment-plan reviews.
  • Maintain shift logs and ensure safety and OSHA compliance; handle sharps and restricted items.
  • Use motivational interviewing to support recovery and community integration; connect to community resources.
  • Provide safe and timely transportation for appointments and activities; cover multiple PACT locations.

What You Will Do:

  • Provide direct and indirect nursing services to individuals served by BHN's Program of Assertive Community Treatment (PACT), collaborating with the multidisciplinary team to support coordinated, person-centered care.
  • Coordinate and complete assessments of individuals' physical-health needs, make appropriate referrals to community medical providers, and integrate medical and psychiatric treatment.
  • Follow Medication Administration Program (MAP) policies and procedures and complete health- and medication-related documentation in accordance with applicable requirements.
  • Under the direction of the team psychiatrist and in collaboration with PACT nurses, manage medications, pharmaceuticals, medical supplies, and related clinical workflows.
  • Support the development, implementation, review, and documentation of treatment plans, health goals, and recovery objectives, providing ongoing information about individuals' progress and needs.
  • Serve as primary case manager for an assigned caseload when requested, completing required assessments, treatment-plan reviews, and financial-management plans.
  • Maintain accurate shift logs and handoffs, account for and secure sharps and other restricted items, follow OSHA requirements, and promptly report safety or maintenance concerns.
  • Use motivational interviewing and recovery-oriented approaches to engage individuals, address ambivalence, promote empowerment and community integration, and support access to community resources.
  • Provide safe and timely transportation in a personal vehicle for medical appointments, community activities, shopping, errands, and other service needs, and provide coverage across PACT locations as assigned.
  • Maintain timely documentation in CareLogic, Smartsheet, and other required systems; participate in meetings and training; work weekends, evenings, or holidays as needed; and complete other assigned duties.

Who You Are:

  • Bachelor's degree in a related field required.
  • Current, unrestricted Registered Nurse license and required nursing certifications required.
  • At least six months of experience in human services preferred.
  • Knowledge of MAP requirements, medication management, physical-health assessment, treatment planning, care coordination, and community-based nursing practice.
  • Commitment to recovery, empowerment, human rights, person-centered care, and the community integration of individuals with complex behavioral-health needs.
  • Ability to work effectively with diverse and challenging populations, including individuals with forensic-system involvement, and willingness to apply the Sequential Intercept Model when relevant.
  • Working knowledge of community resources and demonstrated ability to help individuals access medical, behavioral-health, social, and recovery supports.
  • Strong clinical judgment, organization, time-management, conflict-resolution, teamwork, and verbal and written communication skills.
  • Proficiency with electronic records, internet, email, word processing, agency communication systems, CareLogic, and other required technology.
  • Valid driver's license and a reliable, properly registered and inspected vehicle available during all working hours required, along with the ability to serve as essential staff and meet the physical requirements of the role, including lifting or carrying up to five pounds, with or without reasonable accommodation.
