Job summary

Behavioral Health Network (BHN) seeks a Licensed Practical Nurse for the Adult Community Crisis Stabilization (A-CCS) team to deliver direct clinical and practical nursing services.

You will conduct intake and ongoing assessments, administer medications, document thoroughly, and coordinate with providers, pharmacies, and staff to support treatment planning and continuity of care, while educating clients on recovery and wellness.

Qualifications

  • MA LPN license is required.
  • At least 1 year in behavioral health and 1 year in medical-surgical nursing.
  • Ability to complete nursing assessments and coordinate care.

What You Will Do
  • Provide direct clinical and practical nursing services to adults receiving care through BHN's Adult Community Crisis Stabilization (A-CCS) program.
  • Complete comprehensive intake and ongoing nursing assessments, including review of systems, diagnoses, mental status, vital signs, and emerging medical or behavioral-health needs.
  • Administer medications as ordered, monitor medication effectiveness and safety, and maintain complete and accurate medication records and physician orders.
  • Use nursing assessment and clinical judgment to determine and implement appropriate treatment or referral responses using the least restrictive alternative.
  • Coordinate with the Nurse Manager, providers, pharmacies, emergency departments, program staff, significant others, and collateral partners to support treatment planning and continuity of care.
  • Provide individual and group education about recovery, health, wellness, medical conditions, medications, and treatment efficacy.
  • Build rapport and trust with individuals served while maintaining a safe, healthy, respectful, and recovery-oriented treatment environment.
  • Triage nursing workflow during assigned shifts, participate in daily treatment-planning and disposition meetings, and respond effectively to changing or urgent clinical needs.
  • Complete nursing assessments, progress notes, medication and physician orders, discharge summaries, and other required documentation clearly, accurately, and on time.
  • Facilitate transfers, aftercare planning, and discharge to the next appropriate level of care; maintain required licensure and training; and perform other duties as assigned.
Who You Are
  • High school diploma or GED required.
  • Current, unrestricted Massachusetts Licensed Practical Nurse license required.
  • At least one year of experience or training working in behavioral health and one year of post-graduate medical-surgical nursing experience required.
  • Basic understanding of mental-health conditions and appropriate pharmaceutical, medical, and psychoeducational interventions required.
  • Ability to complete nursing assessments, identify medical and psychiatric needs, administer medications safely, and coordinate appropriate treatment and referrals.
  • Ability to provide high-quality, community-based crisis care and educate individuals about recovery, health, wellness, and medications.
  • Strong clinical judgment, problem-solving, adaptability, organization, time-management, and prioritization skills, including the ability to respond effectively to urgent needs.
  • Demonstrated cultural responsiveness and ability to establish rapport and work respectfully with individuals from diverse backgrounds and with complex needs.
  • Strong interpersonal, teamwork, technology, and verbal and written communication skills, with the ability to maintain timely and accurate electronic documentation.
  • Ability to serve as essential staff and meet the physical requirements of the role, including regular standing, walking, and sitting and lifting or carrying up to 25 pounds, with or without reasonable accommodation.
