Job summary

Behavioral Health Network, Inc. is seeking a Master's Level Clinician to join our Program of Assertive Community Treatment (PACT) team.

This role provides intensive community-based services for individuals with complex behavioral health needs, including those involved with the forensic system, across Western Massachusetts. You will complete evaluations, create treatment plans, coordinate care with families and partners, and help maintain MAP documentation while working toward MAP certification

Qualifications

• Master's degree in Social Work, Psychology, Mental Health Counseling, or a closely related behavioral health field required.
• At least six months of human-services experience preferred; more is valued for complex cases.
• Ability to work with clinically complex, culturally diverse, and forensically involved individuals.
• Knowledge of assessment, treatment planning, crisis evaluation, and community-based care.
• Commitment to empowerment, dignity, safety, and community integration.

Responsibilities

• Provide intensive, community-based clinical services within the PACT model for individuals with complex needs.
• Complete intake evaluations, clinical formulations, risk and crisis assessments, and safety plans.
• Deliver individual, family, couples, and group treatment grounded in recovery-oriented practices.
• Serve as primary clinician and case manager for an assigned caseload with measurable progress.
• Coordinate care with family, hospitals, and community partners; facilitate access to resources.
• Provide outreach in homes, hospitals, and community settings; support independence and integration.
• Support medication-related services and document health and medication information per MAP regulations.
• Maintain timely progress notes, shift reports, and program documentation; participate in meetings and training.

What You Will Do:

• Provide intensive, community-based clinical services within the Program of Assertive Community Treatment (PACT) model for individuals with complex behavioral health needs, including those involved with the forensic system.
• Complete intake evaluations, clinical formulations, recommendations, risk and crisis evaluations, and individualized safety plans using sound clinical judgment.
• Provide individual, family, couples, and group treatment grounded in appropriate clinical theories, principles, and recovery-oriented practices.
• Serve as primary clinician and case manager for an assigned caseload, completing assessments, treatment plans, reviews, financial-management plans, and strategies that support measurable progress.
• Coordinate care with family members, residential providers, hospitals, community partners, and other members of each individual's support network as clinically appropriate.
• Provide frequent outreach in homes, hospitals, residences, and community settings while encouraging independence, informed decision-making, dignity, and community integration.
• Assist with and monitor activities of daily living, connect individuals with community resources, and support access to medical, behavioral health, social, and recovery services.
• Support medication-related services within role expectations and complete health and medication documentation in accordance with Medication Administration Program (MAP) regulations and agency procedures.
• Complete progress notes by the end of each shift and maintain timely treatment, incident, medication-occurrence, shift-report, and program documentation.
• Maintain direct-service and outcome expectations, participate in shift communication, supervision, meetings, training, and continuing education, and follow PACT policies and procedures.
• Provide safe transportation as needed and maintain flexibility to work assigned weekends, second shifts, and holidays based on program needs.

Who You Are:

• Master's degree in Social Work, Psychology, Mental Health Counseling, or a closely related behavioral health field required.
• At least six months of human-services experience preferred; community-based experience with serious mental health needs is strongly valued.
• Ability to work with clinically complex, culturally diverse, and forensically involved individuals within the Sequential Intercept framework.
• Knowledge of clinical assessment, treatment planning, psychotherapy, crisis and risk evaluation, case management, recovery principles, and community-based care.
• Commitment to empowerment, human rights, personal growth, dignity, safety, and community integration for individuals served.
• Working knowledge of community resources and the ability to coordinate services, facilitate access, and collaborate effectively with multidisciplinary partners.
• Excellent clinical judgment, conflict-resolution, interpersonal, verbal, and written communication skills.
• Strong organization, documentation, technology, and follow-through skills, with the ability to manage urgent needs and changing priorities.
• Valid driver's license and a reliable vehicle available during all working hours for community-based services and transportation of individuals served required.
• Ability to obtain and maintain Medication Administration Program (MAP) certification within the first six months of employment.
• Ability to work the assigned essential-staff schedule and meet the role's physical requirements, including lifting or carrying up to 40 pounds, with or without reasonable accommodation.
