Job summary

Behavioral Health Network, Inc. is seeking a Registered Nurse for its Transitional Support Services (TSS) team in Massachusetts. You will provide person-centered nursing care, complete intake assessments, and ensure safe medication administration.

You will collaborate with providers, pharmacies and staff to support recovery, continuity of care and education for patients and families. Strong clinical judgment and documentation skills are essential.

Qualifications

  • Requires Massachusetts RN license and a high school diploma or GED.
  • Experience in behavioral health and post-graduate medical-surgical nursing is preferred.
  • Strong clinical judgment, critical thinking and communication skills are essential.

Responsibilities

  • Provide nursing services to individuals in BHN's TSS program.

What You Will Do
  • Provide compassionate, person-centered clinical and nursing services to individuals receiving care through BHN's Transitional Support Services (TSS) program.
  • Complete comprehensive intake and ongoing nursing assessments, including review of systems, diagnoses, mental status, vital signs, recovery needs, and emerging medical or behavioral-health concerns.
  • Transcribe, verify, and administer medications as ordered; maintain accurate medication records; inventory medications; coordinate refills; and dispose of outdated or unused medications according to program policy.
  • Use nursing assessment and clinical judgment to identify health needs and implement appropriate treatment or referral responses that support safe recovery and continued stabilization.
  • Consult regularly with the Nurse Manager, providers, pharmacies, program staff, significant others, and community partners to coordinate treatment, address medical needs, and promote continuity of care.
  • Provide individuals, groups, families, and staff with education about recovery, health, wellness, medications, self-management skills, and strategies that support long-term progress.
  • Build rapport and trust with individuals served while maintaining a safe, healthy, respectful, and recovery-oriented milieu with appropriate professional boundaries.
  • Triage nursing workflow during assigned shifts, participate in treatment-planning and transition meetings, and respond effectively to changing or urgent clinical needs.
  • Complete nursing assessments, progress notes, medication orders, discharge summaries, and other required clinical documentation clearly, accurately, and on time.
  • Support aftercare planning and transitions to community-based services or the next appropriate level of care; maintain required licensure and continuing education; and perform other duties as assigned.
Who You Are
  • High school diploma or GED required.
  • Current, unrestricted Massachusetts Registered Nurse license required.
  • At least one year of experience or training working in behavioral health and one year of post-graduate medical-surgical nursing experience required.
  • Basic understanding of behavioral-health conditions, recovery principles, co-occurring needs, and appropriate pharmaceutical, medical, and psychoeducational interventions required.
  • Ability to complete comprehensive nursing assessments, identify medical and psychiatric needs, administer medications safely, and coordinate appropriate treatment and referrals.
  • Ability to provide high-quality transitional and recovery-focused care and educate individuals about health, wellness, medications, self-management, and community supports.
  • Strong clinical judgment, critical thinking, problem-solving, adaptability, organization, time-management, and prioritization skills, including the ability to respond effectively to urgent needs.
  • Demonstrated cultural responsiveness and ability to establish rapport and work respectfully with individuals from diverse backgrounds and with complex needs.
  • Excellent interpersonal, teamwork, technology, and verbal and written communication skills, with the ability to maintain timely and accurate electronic documentation.
  • Ability to serve as essential staff and meet the physical requirements of the role, including regular standing, walking, and sitting and lifting or carrying up to 25 pounds, with or without reasonable accommodation.
