Job summary

Miami Jewish Health is seeking an experienced RN PACE to provide nursing care to participants enrolled in Florida PACE Centers. Under supervision of the clinic manager RN and medical director, you will develop care plans and coordinate with a multidisciplinary team.

Responsibilities include comprehensive nursing histories, triaging, referrals to medical directors, and active participation in IDT and family conferences. Florida license required; bilingual English/Spanish a plus.

Qualifications

• RN license in Florida is required.
• Minimum 1 year RN experience or 3 years LPN experience.
• Experience working with frail/elderly populations is preferred.

Responsibilities

• Perform comprehensive nursing history and physical on new referrals.
• Create care plans and participate in IDT.
• Evaluate and triage participants with episodic illnesses.
• Refer participants to medical director or specialists as indicated.
• Participate in family conferences and interdisciplinary team meetings.
