Job summary

Miami Jewish Health seeks an experienced RN for its Florida PACE Centers. Under the supervision of the clinic manager RN and medical director, you will provide nursing care to all enrolled participants, developing care plans and coordinating with IDT.

The role requires a Florida RN license, BLS, and at least 1 year of RN experience with frail/elderly populations; bilingual English/Spanish helpful. The position is on-site in Fort Lauderdale with competitive benefits.

Qualifications

Graduate from an accredited school of nursing.
RN license (FL) required.
BLS certification required; ACLS preferred.
RN with at least 1 year of experience, or LPN with 3+ years.
Experience with frail/elderly populations preferred.

Responsibilities

Performs comprehensive nursing history and physical on new referrals.
Creates appropriate care plan and participates in IDT.
Evaluates and triages participants with episodic illnesses.
Refers participants to medical director or medical specialist as indicated.
Actively participates in family conferences.
Participates in interdisciplinary team meetings, committees and work groups as assigned.
