Miami Jewish Health is seeking an LPN for the PACE program to provide nursing care under RN supervision for assigned participants. The role includes medication administration, IV therapy initiation, and patient/family education within a multidisciplinary team.

Education requirements include a Florida LPN license, a high school diploma or equivalent, and a valid Florida driver’s license. The position emphasizes teamwork, geriatrics care, and bilingual communication in English/Spanish as a plus.

Qualifications

• High School Diploma or equivalent.
• Graduate of an accredited school of practical nursing.
• Current licensure by the Florida State Board of Nursing.
• Valid Florida Driver's License & Proof of Insurance

Responsibilities

• Performs care for assigned participants per established department guidelines, policies and procedures, MD orders and/or coordinates under the direction of an RN.
• Administers medications and initiates and maintains IV therapy according to nursing rules and procedures.
• Counsels or teaches patients/families as planned by RN.
• Communicates patient care data to other caregivers according to guidelines of reporting.
• Documentation is kept according to established standards.
• Actively participates as a member of the Interdisciplinary Team.
• Performs departmental activities in compliance with department, regulatory, and professional standards, and guidelines
