Job summary

Miami Jewish Health in Florida seeks a Nurse Practitioner for its PACE program. You will provide direct primary medical care and counseling to participants under the supervision of the medical director, coordinating with the care team to manage chronic and acute conditions.

Responsibilities include comprehensive H&P on new referrals, ongoing care in home and nursing home settings, and active participation in family conferences and interdisciplinary meetings. Florida ARNP/PA licensure required.

Qualifications

• Graduate from an accredited nursing program.
• Active ARNP/PA license in Florida.
• Minimum 1 year experience with frail elderly population.

Responsibilities

• Performs comprehensive history and physical on new referrals.
• Manages acute, episodic, and chronic illnesses for participants.
• Coordinates discharge planning with the multidisciplinary team.
• Maintains patient charts according to department guidelines.
• Participates in family conferences and team meetings.
