Job summary

Miami Jewish Health is seeking an experienced RN PACE to provide nursing care to participants enrolled in Florida PACE Centers. Under supervision of the clinic manager RN and medical director, you will develop care plans and coordinate with a multidisciplinary team.

Responsibilities include comprehensive nursing histories, triaging, referrals to medical directors, and active participation in IDT and family conferences. Florida license required; bilingual English/Spanish a plus.

Qualifications

• RN license in Florida is required.
• Minimum 1 year RN experience or 3 years LPN experience.
• Experience working with frail/elderly populations is preferred.

Job Summary

Under the general supervision of the clinic manager RN and medical director of FLORIDA PACE CENTERS, provides nursing care to all persons enrolled in the program.

Essential Job Functions

• Performs comprehensive nursing history and physical on new referrals.
• Creates appropriate care plan and participates in IDT.
• Evaluates and triages participants with episodic illnesses
• Refers participants to medical director or medical specialist as indicated
• Actively participates in family conferences.
• Participates in interdisciplinary team meetings, committees and work groups as assigned.

Job Requirements

Education

• Graduate from an accredited school of nursing

Experience

• At least 1 year of RN or 3 years of LPN required
• Minimum one-year experience working with a frail/elderly population

Licenses/Certifications

• Current RN license by the Florida State Board of Nursing
• Valid Florida Driver’s License & Proof of Insurance

Abilities Required

• Bilingual English/Spanish, knowledge of Creole a plus
• Proven clinical leadership
• Ability to get along with others in an interdisciplinary team environment

• Ability to assist transferring patients and ambulation which may involve lifting, pushing, and pulling of patients from 50-150 pounds
