The Senior Community Care of Maryland PACE in Lanham, MD is seeking a Registered Nurse to perform initial assessments in clinic and patient homes, coordinate with an Interdisciplinary Team, and conduct periodic re-assessments. The role supports elderly participants in a comprehensive PACE program, with a focus on individualized care plans and collaboration.

Qualifications include an RN license, CPR certification, and at least one year of geriatric care experience.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current state RN license required.
• CPR certification required.

Responsibilities

• Conducts in-person comprehensive initial nursing assessments in the PACE clinic and in participants' homes.
• Coordinates with Interdisciplinary Team to develop a comprehensive plan of care.
• Performs in-person nursing re-assessments in the PACE clinic and in participants' homes semi-annually and as needed.
