Job summary

The RN will perform initial assessments and care planning with the Interdisciplinary Team, coordinate in-home visits, and participate in on-call rotations while growing with a mission-driven organization.
Qualifications
- Minimum of 1 year experience working with frail/elderly populations.
- Graduate of an accredited professional nursing program.
- Current RN license and CPR certification required; valid driver's license.
Responsibilities
- Conducts in-person initial nursing assessments in the PACE clinic and in participants' homes.
- Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care.
- Performs semi-annual nursing re-assessments and participates in the on-call rotation.
- Maintains regular attendance at Interdisciplinary Team meetings and communicates participant changes.
