Senior Community Care of Maryland PACE in Lanham, MD invites a dedicated Registered Nurse to join our on-site team. This role offers a meaningful career helping seniors live safely and with dignity, in a supportive healthcare setting.

The RN will perform initial assessments and care planning with the Interdisciplinary Team, coordinate in-home visits, and participate in on-call rotations while growing with a mission-driven organization.

Qualifications

  • Minimum of 1 year experience working with frail/elderly populations.
  • Graduate of an accredited professional nursing program.
  • Current RN license and CPR certification required; valid driver's license.

Responsibilities

  • Conducts in-person initial nursing assessments in the PACE clinic and in participants' homes.
  • Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care.
  • Performs semi-annual nursing re-assessments and participates in the on-call rotation.
  • Maintains regular attendance at Interdisciplinary Team meetings and communicates participant changes.

Pay Range: $44.70-$51.40
Schedule: Monday-Friday

What You Bring (Requirements)

  • A minimum of one year’s experience in working with the frail or elderly population required.
  • Graduate of an accredited school of professional nursing
  • Current state Registered Nurses (R.N.) License, Cardio Pulmonary Resuscitation (CPR) certification required
  • Must have a valid driver’s license, proof of insurance and have means of transportation.

What You’ll Be Responsible For:

Conducts an in-person comprehensive initial nursing assessments in the PACE clinic and in participant’s homes with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.

  • Conducts an in-person nursing re-assessments in the PACE clinic and in participant’s homes semi annually and as needed.
  • Participates in the nursing on-call rotation.
  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
