Job summary

Volunteers of America National Services is seeking a Registered Nurse in Lanham, MD to deliver compassionate nursing care within the PACE program. This on-site role focuses on initial assessments, care planning, and coordinating with an Interdisciplinary Team to ensure comprehensive 24-hour care for participants.

The ideal candidate will have at least one year of experience with frail or elderly populations, hold a valid RN license, CPR certification, and a driver's license with reliable

Qualifications

  • Minimum of one year’s experience with frail or elderly populations.
  • Graduate of an accredited school of professional nursing.
  • Current state RN license and CPR certification.
  • Valid driver’s license and reliable transportation.

Responsibilities

  • Conducts in-person nursing assessments in the PACE clinic and participants' homes to develop a plan of care.
  • Performs semi-annual re-assessments and participates in on-call rotation.
  • Collaborates with Interdisciplinary Team to coordinate 24-hour care delivery.

Pay Range: $44.70-$51.40
Schedule: Monday-Friday

What You Bring (Requirements)

  • A minimum of one year’s experience in working with the frail or elderly population required.
  • Graduate of an accredited school of professional nursing
  • Current state Registered Nurses (R.N.) License, Cardio Pulmonary Resuscitation (CPR) certification required
  • Must have a valid driver’s license, proof of insurance and have means of transportation.

What You’ll Be Responsible For:

Conducts an in-person comprehensive initial nursing assessments in the PACE clinic and in participant’s homes with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.

  • Conducts an in-person nursing re-assessments in the PACE clinic and in participant’s homes semi annually and as needed.
  • Participates in the nursing on-call rotation.
  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
