Cherry Blossom PACE in Alexandria, VA is seeking a skilled Registered Nurse to join our inter-disciplinary team. The RN will provide comprehensive clinical care, coordinate services, and perform intake assessments for PACE participants and potential enrollees across clinic, home, and community settings.

You will ensure compliance with CMS and state regulations, maintain high-quality nursing care, and collaborate with physicians, social workers, and field staff.

Qualifications

• Active RN license in good standing.
• Current CPR certification.
• Valid driver’s license for field work.
• Experience in geriatric, home health, or PACE preferred.
• Bilingual in Spanish or Amharic preferred.
• Ability to work across clinic and community settings.

Responsibilities

• Provide comprehensive clinical care and care coordination.
• Conduct field-based assessments for participants and enrollees.
• Work as part of the Interdisciplinary Team (IDT) to support health outcomes.
• Ensure compliance with federal, state, CMS PACE regulations.

About the RoleJob Summary:

The PACE Registered Nurse provides comprehensive clinical care, care coordination, field-based services, and intake assessments for PACE participants and prospective enrollees. This role functions as an integrated member of the Interdisciplinary Team (IDT), supporting participant health outcomes through assessment, care planning, coordination, education, and hands‑on nursing services across clinic, home, and community settings.

The RN ensures compliance with federal, state, and CMS PACE regulations while delivering participant‑centered, high‑quality nursing care throughout the participant lifecycle—from intake through ongoing enrollment.
