Job summary

Providence Health and Services seeks a Community Care RN to support and supervise nursing care for participants in the community, based on the nursing process and Oregon rules. The role spans clinics, home visits and community services, aligned with the PACE program to keep elders healthy and independent.

Required: Oregon RN license on hire, BLS within 30 days, 2 years RN experience in PACE or related fields.

Qualifications

• Oregon Registered Nurse license required at hire.
• National Provider BLS - American Heart Association within 30 days of hire.
• 2 years RN experience in PACE, home health, SNF, community health, hospice, or related field.

Responsibilities

• Provide and supervise professional nursing care for Providence participants in the community.
• Apply nursing process: assess, plan, order, administer, delegate, teach and supervise care delivery.

: Monday to Friday Day shiftThe Community Care RN provides and supervises professional, independent nursing care for Providence participants in the community. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. Applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients' optimal health and independence.Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more. Required Qualifications:

• Upon hire: Oregon Registered Nurse License
• Within 30 days of hire: National Provider BLS - American Heart Association
• 2 years RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience.

Preferred Qualifications:

• Bachelor’s Degree in Nursing
• Upon hire: Current recognition by a national professional organization in care management, geriatrics or hospice
