Job summary

Providence is seeking a Registered Nurse Care Manager for the PACE program in the Seattle area. The role involves conducting patient assessments, developing and implementing care plans, and coordinating care with interdisciplinary teams to ensure medically and financially appropriate care for elderly participants living in the community.

Qualifications

  • RN licensure required (WA)
  • BLS certification (AHA) required
  • 1 year LTC or geriatric nursing experience preferred
  • Bachelor’s degree in Nursing preferred

Responsibilities

  • Perform patient assessments and develop care plans.
  • Coordinate and monitor care across disciplines to ensure appropriate medical and financial care.
  • Manage care for participants recently discharged from hospital or skilled nursing facilities.
  • Collaborate with IDT Social Worker and Physician to gather medical and social information for safe, comprehensive decisions.

Schedule

Monday-Friday Day shift

The Registered Nurse Care Manager establishes a well-rounded care practice that includes: a patient assessment, care planning, implementation, coordination, and monitoring and ongoing routine evaluation that ensures medically and financially appropriate care. Responsibilities include: facilitating interdisciplinary team care coordination, intensive medical management of participants recently discharged from hospital and skilled nursing facilities. Additionally, this position will work closely with the IDT Social Worker and Physician to collect necessary medical and social information to ensure comprehensive decisions are made which ensure the safety and well being of participants.

Required Qualifications
  • Education sufficient for clinical licensure.
  • Upon hire: Washington Registered Nurse License
  • Upon hire: National Provider BLS - American Heart Association
  • 1 year Long term care or geriatric nursing.
Preferred Qualifications
  • Bachelor’s Degree in Nursing
  • Medical surgical or rehabilitation medicine.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
