Job summary

Providence Health & Services is seeking a Community Care RN to provide and supervise nursing care in the community, including home visits and clinics at the Irvington site. You will apply the nursing process in alignment with the Oregon Nurse Practice Act to promote patient health and independence.

The role supports Providence PACE, a program of all-inclusive care for the elderly, and offers a comprehensive benefits package and growth opportunities within a large faith-based health system.

Qualifications

• Oregon RN license required at hire.
• BLS certification within 30 days of hire.
• 2 years RN experience in PACE, home health, SNF, community health, hospice nursing or related fields.

Responsibilities

• Provide and supervise professional nursing care for Providence participants in the community.
• Utilize nursing process to assess, plan, implement and evaluate care.
• Support elders to live active, independent lives within Providence PACE.

This position will be supporting our Irvington site.

Schedule : Monday to Friday Day shift

The Community Care RN provides and supervises professional, independent nursing care for Providence participants in the community. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. Applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients' optimal health and independence.

Required Qualifications:

• Upon hire: Oregon Registered Nurse License
• Within 30 days of hire: National Provider BLS - American Heart Association
• 2 years RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience.

Preferred Qualifications:

• Bachelor's Degree in Nursing
• Upon hire: Current recognition by a national professional organization in care management, geriatrics or hospice

Pay Range: $44.58 - $69.21
