Job summary

The Providence Health Plan Group is seeking a Community Care RN to support the Irvington site in Portland, OR. The role provides and supervises professional nursing care for Providence participants in the community, following the nursing process to promote health and independence.

Qualified candidates must hold an Oregon RN license, obtain BLS within 30 days, and have 2 years RN experience in PACE, home health, or related settings. A BSN and national care management recognition are preferred.

Qualifications

• Oregon RN license required at hire.
• BLS within 30 days of hire.
• 2 years RN experience in PACE, home health, SNF, community health, hospice, or related field.

Responsibilities

• Provide and supervise professional nursing care per the nursing process.
• Apply assessment, planning, delegation, teaching, and supervision in community settings.
• Support Providence PACE program and empower elders to live independently.

Schedule

Monday to Friday Day shift

The Community Care RN provides and supervises professional, independent nursing care for Providence participants in the community. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. Applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients' optimal health and independence.

Required Qualifications:

• Upon hire: Oregon Registered Nurse License
• Within 30 days of hire: National Provider BLS - American Heart Association
• 2 years RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience.

Preferred Qualifications:

• Bachelor’s Degree in Nursing
• Upon hire: Current recognition by a national professional organization in care management, geriatrics or hospice
