Providence Health & Services in Renton, WA is seeking a Flex RN to deliver case management and clinical nursing services for frail elderly participants in a PACE program. Responsibilities include care coordination, hands-on nursing, and coverage for PACE RN Case Manager, clinic, and home health positions.

The role supports an interdisciplinary team with a focus on wrap-around care, including coordination of services, triage, and direct patient care in clinical and home settings.

This is a flex nursing role which provides case management, home health and clinic visits in South King county.

Schedule

Weekdays Monday to Friday Day shift. No weekends or holidays!

As a member of the Interdisciplinary Team, the Registered Nurse (RN) participates in the development of an integrated plan of care and provides direct nursing care to our frail elderly participants. The Flex RN position supports and provides coverage for the PACE RN Case Manager position and PACE clinic nurse and home health nurse positions. This dynamic position utilizes the full-spectrum of high level nursing skills-- from critical -thinking care coordination and triage skills, to hands‑on, clinical nursing skills. Required Qualifications:

• Education sufficient for clinical licensure.
• Upon hire: Washington Registered Nurse License
• Upon hire: National Provider BLS - American Heart Association
• 1 year Long term care or geriatric nursing.

Preferred Qualifications:

• Bachelor's Degree in Nursing
• Medical surgical or rehabilitation medicine.
