Responsibilities

• Coordinate patient care across the cancer continuum with multiple departments.
• Document assessments, education, referrals, and care coordination in the EMR.
• Facilitate multidisciplinary care conferences and review meetings.
• Educate patients and families and guide them to resources and services.
• Collaborate with physicians and professionals across locations to ensure timely care.
