Job summary

Qualifications

• Graduate of an accredited nursing program.
• Wisconsin RN license (or compact); maintain WI residency for compact.
• CPR/BLS certification required; AAON certification required within applicable timeframe.

Responsibilities

• Support oncology navigation program goals and onboarding of navigators.
• Collaborate with providers to develop standardized protocols and workflows.
• Assess barriers to care and address social determinants of health.
• Coordinate care between departments and advocate for patient-centered care.
• Participate in tumor boards and community outreach for education.
• Document care plans in the EMR and support transitions of care.
