Job summary

Responsibilities

• Facilitates coordination of patient care services to ensure excellence in patient care and flow; completes needs assessments and follows patients through the care continuum.
• Documents all patient assessments, education, referrals, and other activity in the EMR.
• Facilitates development and adherence to clinical guidelines to ensure optimal outcomes and coordinates multidisciplinary conferences.
